7v7 Indoor Soccer Tournament — Team Registration Agreement & Waiver
 

Venue: KK Athletics 

Organizers: Jaycutz & Aldrich Sports League (“ASL”)

Team Fee: $550 per team

1) Registration & Payment

 

  • The team captain is the authorized representative for the team.

  • $550 is due via Zelle to Jason (631) 902-9701.

  • If not paid in advance, captain agrees to pay $550 in full on event day (Zelle or cash) before the team’s first match.

  • Fees are non-refundable except if the event is cancelled by the organizer.

 

 

2) Roster & Format

 

  • 7 players on the field (incl. GK) at a time; max 3 substitutes.

  • Roster cap = 10 total players per team.

  • Players may appear on only one team roster.

 

 

3) Equipment & Attire

 

  • Indoor soccer footwear only (non-marking flats or turf). No metal studs.

  • Shin guards strongly recommended.

  • Teams should wear matching shirts; pinnies may be provided if needed.

 

 

4) Conduct & Sportsmanship

 

  • Play respectfully; zero tolerance for abusive language, fighting, or harassment of officials/players/staff.

  • Organizers/venue may remove any player/team for unsafe or disrespectful behavior without refund.

  • Officials’ and organizers’ decisions are final.

 

 

5) Assumption of Risk; Release & Indemnity

 

I understand that indoor soccer carries inherent risks (collisions, falls, sprains/strains, illness, property damage). In consideration of participation, I assume all risks and, to the fullest extent permitted by law, release and hold harmless Jaycutz, Aldrich Sports League (ASL), KK athletics and their owners, officers, employees, volunteers, contractors, sponsors, and agents from any and all claims, liabilities, damages, or losses arising from participation, including injury, illness, or property damage. I agree to indemnify and defend the Released Parties against claims arising from my (or my team’s) participation or conduct.

 

 

6) Medical Authorization

 

In case of injury/emergency, I authorize the organizer/venue to secure medical care as deemed necessary. I am responsible for all associated costs.

 

 

7) Media & Publicity

 

Event activities may be photographed, filmed, or live-streamed. I grant Jaycutz, ASL, and Hamptons Field House the right to record and use my/team name, image, and likeness for event coverage and promotion without further approval or compensation.

 

 

8) Scheduling & Rules

 

Organizer may adjust schedules, format, or rules for safety, fairness, or venue needs.
